Licensed Practical Nurse (LPN) - Wound Care Clinic

We are South City Hospital. Our primary function is to offer continuous nursing, medical, and other health and social services on a 24-hour basis, under physician directed care and RN supervision.

We focus on providing care for patients who are not in the acute phase of illness or requiring services in a hospital and who require primary, restorative, or skilled nursing services on an inpatient or custodial care basis. Our goal is to help our patients reach a degree of self-care in essential daily living.

The Licensed Practical Nurse is responsible for the delivery of quality patient care, using a holistic approach, through the nursing process of assessment, planning, implementation and evaluation under the direction of a Registered Nurse.  The Licensed Practical Nurse functions within the limits of the Missouri Practice Act, South CityHospital and Patient Care Services policies and practice guidelines, and performs in a comparable manner to peers with similar training, experience and circumstances.


  • Assists with the assessment of the physical and psychosocial dimensions of the patient, using a holistic approach, and under the direction of a Registered Nurse, within the scope of knowledge and experience base, upon admission and throughout the nursing process.  Documents appropriately into the patient record following established standards of the patient care services department.
  • Makes recommendations toward the development of a formal written individualized plan of care.  Assists in its initiation, implementation and evaluation under the direction of a Registered Nurse within the appropriate time frame based on the nursing assessment and diagnosis and appropriate Standards of Care.
  • Provides quality patient care from admission to discharge by collaborating with all healthcare team members and other departments and maintaining cost-effectiveness in accordance with established policies and practice guidelines.
  • Provides IV Therapy within guidelines of Missouri State Board of Nursing and South City Hospital Policies and Procedures.
  • Participates in unit based quality improvement processes as required.
  • Assumes accountability for delivery of care through effective communication with all available resources in accordance with established policies and practice guidelines.
  • Assumes accountability for professional growth by participating in continuing education and in-service programs.  Accepts reassignment to other areas/shifts and participates in the orientation program as required.
  • Demonstrates competencies of the Missouri Nurse Practice Act by practicing within established policies and practice guidelines.
  • Demonstrates unit-defined competencies as required.
  • Performs other duties as assigned.


  • Graduate from a Licensed Practical Nursing Program.
  • Eligible for or currently licensed as a Licensed Practical Nurse in the State of Missouri.
  • Certification in IV Therapy for Med Surg; wound care experience preferred.  
  • Recent hospital experience preferred.
  • Completion of CPR.
  • Evaluative skills necessary for patient assessment.
  • Decision-making skills required for treatment planning and implementation.
  • Clinical reasoning and judgment skills necessary for evaluation of nursing interventions and for safety of patients and self.
  • Excellent communication skills to interact verbally with patients, visitors and staff.  Ability to recognize need for and alter communication with patients with communication impairments.
  • Ability to manage time, organize daily schedule to meet productivity standards and cope with professional stress.

Monday through Friday, Day Shift.
